When Hip Pain Needs Assessment Rather Than Self-Management

A structured self-management programme can be appropriate for many stable hip problems, but the first decision should always be whether it is safe to self-manage.
After a fall or significant injury
Severe hip pain after a fall or injury, especially when you cannot walk or put weight through the leg, requires urgent assessment. Numbness or loss of feeling following an injury also needs urgent review.
Hot, swollen or systemically unwell
A suddenly hot or swollen hip, skin colour change around the area, or hip pain combined with fever or feeling generally unwell should be assessed promptly because infection and other medical causes need to be excluded.
Persistent or recurring hip pain
Professional assessment is sensible when pain keeps returning, progressively worsens, affects sleep or normal activities, or has not improved after a period of appropriate self-management.
Complexity can change the route
Previous surgery, major mobility limitation, significant falls risk, neurological symptoms or multiple health conditions can make a clinician-led or supported pathway more appropriate than a generic online programme.
